Review Form Builder Guide: Templates, Question Types & the 2-3 Question Rule

Master ReviewGen.AI's form builder with 8 industry templates, 5 question types, and the proven 2-3 question rule that increases completion rates by 40-60%.

Your review form determines whether customers complete or abandon the process. Too many questions and they quit. Wrong question types and they get confused. No template and you're guessing. This guide shows you how to build high-converting forms using ReviewGen.AI's templates, question types, and proven design rules.

The 2-3 Question Rule

Before diving into templates and question types, understand this: completion rate drops 15-20% for every additional question after the third.

  • 2 questions: 60-70% completion rate
  • 3 questions: 45-55% completion rate
  • 4 questions: 30-40% completion rate
  • 5+ questions: Below 30% completion rate

The ideal review form has 2 questions:

  1. Star rating (required)
  2. Open-ended feedback (optional)

Add a third question only if you need specific data that improves review quality. Examples: service provider name, appointment date, product purchased, or location visited.

Don't ask for email, phone number, or other contact info unless you have a specific follow-up workflow. Most customers abandon forms that feel like lead capture.

8 Industry Templates

ReviewGen.AI includes pre-built templates for common industries. Each template follows the 2-3 question rule and uses appropriate question types for that business type.

Restaurant Feedback (2 Questions)

Best for: Restaurants, cafes, food trucks, catering
Questions:

  • How would you rate your overall experience? (Rating, required)
  • What did you enjoy most about your visit? (Textarea, optional)

This template captures star rating and highlights. The open-ended question prompts customers to mention specific dishes, service quality, or atmosphere — all details that make reviews more credible and helpful.

Patient Feedback (3 Questions)

Best for: Medical practices, dental offices, chiropractic, therapy, veterinary
Questions:

  • How would you rate your visit? (Rating, required)
  • Was the staff helpful and professional? (Rating, required)
  • Any additional comments or suggestions? (Textarea, optional)

Healthcare forms benefit from the third question because staff professionalism is a key driver of patient reviews. Separating facility rating from staff rating gives more granular feedback and helps the AI generate specific review content.

Store Experience (2 Questions)

Best for: Retail stores, boutiques, convenience stores, grocery
Questions:

  • Rate your shopping experience (Rating, required)
  • Any feedback for us? (Textarea, optional)

Simple and fast. Retail customers are typically on the go, so minimal friction is critical. This template captures star rating and open feedback without slowing them down.

Salon & Spa (2 Questions)

Best for: Hair salons, nail salons, spas, barbershops, beauty services
Questions:

  • How would you rate your service? (Rating, required)
  • Would you book with us again? (Radio, required)

The second question uses radio buttons (Yes/No) instead of open text. This works because booking intent is a simple binary signal. The AI uses the rating plus intent to generate appropriate review tone — enthusiastic for 5-star + Yes, measured for lower ratings.

Auto Service (3 Questions)

Best for: Auto repair, oil change, detailing, car wash, tire shops
Questions:

  • How would you rate our service quality? (Rating, required)
  • Was the work completed on time? (Radio, required)
  • Any comments or concerns? (Textarea, optional)

Timeliness matters in automotive. The second question captures on-time delivery as a distinct metric. This template works well because customers care about both quality and speed.

Real Estate (2 Questions)

Best for: Real estate agents, brokers, property management
Questions:

  • How would you rate your experience? (Rating, required)
  • Was your agent responsive and helpful? (Rating, required)

Real estate reviews focus heavily on agent performance. This template separates overall experience from agent responsiveness, giving the AI two data points to generate specific, credible review content.

Fitness Center (2 Questions)

Best for: Gyms, yoga studios, CrossFit boxes, personal training
Questions:

  • How would you rate our facilities? (Rating, required)
  • Would you recommend our gym? (Radio, required)

Short and conversion-focused. The recommendation question (Net Promoter Score style) works well because fitness businesses thrive on referrals. The AI uses both facility rating and recommendation intent to craft review tone.

Hotel & Hospitality (3 Questions)

Best for: Hotels, motels, vacation rentals, bed & breakfast
Questions:

  • How would you rate your stay? (Rating, required)
  • Rate the cleanliness of your room (Rating, required)
  • What did you enjoy most about your stay? (Textarea, optional)

Hospitality benefits from the three-question structure. Overall stay and cleanliness are distinct factors customers evaluate. The open-ended third question captures highlights like location, amenities, or staff service.

How to Use Templates

Open your review form editor and click the "Form" tab. You'll see a "Templates" button. Click it to browse the 8 pre-built templates.

  1. Find your industry category
  2. Review the template questions
  3. Click "Use This Template"
  4. The questions load into your form
  5. Customize as needed
  6. Click "Save"

Templates replace your current questions, so if you have custom questions you want to keep, write them down before switching.

Don't see your industry? Start with the closest match and customize. For example, a pet grooming business could start with the Salon & Spa template and adjust the wording.

5 Question Types

ReviewGen.AI supports five question types. Each serves a different purpose. Use the right type for the data you need.

1. Rating (Star Rating)

What it is: 1-5 star input
When to use it: Overall satisfaction, specific attribute ratings (cleanliness, quality, staff)
Best practices:

  • Always include at least one rating question (required)
  • Use 5-star scale (industry standard)
  • Label clearly: "Rate your experience" not "Stars?"
  • Place rating questions first

Example questions:

  • How would you rate your overall experience?
  • Rate the quality of service
  • How would you rate our facilities?

2. Text (Single Line)

What it is: Single-line text input
When to use it: Short answers like name, product purchased, service provider, appointment date
Best practices:

  • Use for data that's 1-5 words
  • Add placeholder text as examples
  • Make optional unless critical

Example questions:

  • Your name (optional)
  • Which product did you purchase?
  • Who was your service provider?

3. Textarea (Multi-Line)

What it is: Multi-line text box for longer responses
When to use it: Open-ended feedback, experience descriptions, suggestions
Best practices:

  • Always make optional
  • Use prompts that encourage specific feedback: "What did you enjoy most?" beats "Any comments?"
  • Place after rating questions

Example questions:

  • What did you enjoy most about your visit?
  • Any feedback or suggestions?
  • Tell us about your experience

4. Radio (Single Choice)

What it is: Multiple choice, select one option
When to use it: Yes/No questions, single-select categories (location, product type, service level)
Best practices:

  • Keep options to 3-5 maximum
  • Use for simple binary or category questions
  • Make required if you need this data point

Example questions:

  • Would you book with us again? (Yes / No / Maybe)
  • Was the work completed on time? (Yes / No)
  • Which location did you visit? (Downtown / Westside / Eastside)

5. Checkbox (Multiple Choice)

What it is: Multiple choice, select multiple options
When to use it: Services received, product categories, features used
Best practices:

  • Use sparingly — adds cognitive load
  • Keep options to 4-6 maximum
  • Make optional unless critical

Example questions:

  • Which services did you receive? (Haircut / Color / Blowout / Treatment)
  • What did you order? (Appetizer / Main / Dessert / Drinks)

Warning: Checkbox questions lower completion rates because they require more thought. Only use when you need multi-select data that significantly improves review specificity.

Building Custom Forms

If templates don't fit, build your own form from scratch. Click "Add Question" in the Form tab.

Step-by-step custom form:

  1. Start with rating: Add a rating question, make it required, order it first
  2. Add one context question (optional): Text or radio question for specific data
  3. Add open feedback (optional): Textarea for comments
  4. Stop there: Resist adding more

Question order matters:

  1. Rating questions first (easiest to answer)
  2. Simple questions next (text, radio)
  3. Open-ended questions last (textarea)

This order minimizes abandonment. Customers commit by rating first, then answer follow-ups. If they quit mid-form, you still captured the star rating.

Required vs Optional

Here's the rule: only the star rating should be required. Everything else should be optional.

Why? Required fields create friction. Every required field adds 10-15% abandonment risk. Customers who want to give detailed feedback will fill optional fields. Those who don't will still give you a star rating.

Exception: If you need specific data for AI quality (service provider name, product purchased), make that one field required. But limit required fields to 2 maximum.

Testing Your Form

After building your form, test it before sharing with customers. Click "Preview" or copy your review link into a new tab.

Walk through as a customer:

  • Answer each question
  • Click "Generate Review"
  • Check if the AI-generated draft makes sense
  • Verify the tone and length match your settings
  • Confirm redirect lands on your review platform

Test on mobile. Most customers will complete forms on phones. Verify all question types work smoothly on small screens.

Common Mistakes to Avoid

Too Many Questions

The biggest mistake. If you're asking 5+ questions, cut ruthlessly. Ask: "Does this question improve the AI-generated review?" If no, delete it.

Unclear Wording

"Rate us" is vague. "How would you rate your overall experience?" is clear. Customers should understand what you're asking without re-reading.

All Required Fields

Making every question required tanks completion rates. Only the star rating should be required.

Wrong Question Type

Using textarea for name or text for detailed feedback. Match the question type to the data you need: short = text, long = textarea, binary = radio, multi-select = checkbox.

Leading Questions

"What did you love about our amazing service?" is leading. "What did you think of our service?" is neutral. Let customers decide the tone.

Frequently Asked Questions

How many questions should my review form have?

2-3 questions maximum. Forms with 2 questions see 60-70% completion rates, 3 questions see 45-55%, and 4+ questions see completion rates drop below 30%. Start with just star rating plus one open-ended question.

What's the difference between text and textarea question types?

Text creates a single-line input field for short answers (name, phone, product name). Textarea creates a multi-line box for longer responses (feedback comments, experience descriptions). Use text for simple data, textarea for detailed feedback.

Should all questions be required?

No. Only make the star rating required. Make every other question optional. Required fields create friction and lower completion rates. Customers who want to give detailed feedback will fill out optional fields. Those who don't will still complete the rating.

Can I use the same form for multiple businesses?

No. Each form belongs to one business profile. Your business profile (name, description, industry) feeds the AI review generation. If you manage multiple businesses, create a separate business profile and form for each.

How do I change from one template to another?

Click "Use Template" to load a new template. This replaces all existing questions. If you have custom questions you want to keep, write them down before switching templates. You can manually add them back after.
